There are four components of Service Link. Below the four components are outlined. Further down, there is an example of the Student Referral Form.
Student Search
Allows users to search for a student record AND/OR create a new referral for a student
Referral
Allows users to search for existing/created referrals for specific service providers AND/OR create a new referral for a student to a service provider.
Family Engagement
Allows users to create an event that a student or groups of students may be “referred” to. These events are associated with Family Engagement activities.
File Export
Allows a coordinator to download all student, series and event referral data into a standardized report necessary for Santa Clara County Behavioral Health Services Department. The referral information includes referrals since July 1st.
